How to Enroll

After learning and making decisions, it's time to enroll:

Generally, any elections you make for 2025 will remain in effect through Dec. 31, 2025.* When your enrollment period ends, you may only make a change to your benefit elections during the year if you experience a qualified life event qualified life event (such as a marriage or divorce, birth or adoption of a child, etc.) and report it through MyHR within 30 calendar days of a life event.

If you do not change your benefit elections on MyHR within 30 calendar days of a qualified life event, no changes will take effect.
How to enroll:Visit MyHR before the deadline.Visit MyHR through Connect within 30 calendar days of your life event.
When you will see your elections summary:

At the end of your enrollment process, you will see your elections summary. Make sure you carefully review it for accuracy at that time.

 

If you find any inconsistencies, you should notify the People Experience Center immediately via ERIN and within four days from the end of enrollment period. “Request Assistance” directly within ERIN and a member of the People Organization will contact you directly.

*If you’re a participant in the Green or Orange health plan, you’re required to enroll in the HSA in order to complete the enrollment process; however, you’re not required to make personal contributions and you’ll receive the company contribution.

**You have 15 calendar days from your hire date or the date you become a benefits-eligible employee to enroll in your Health and Welfare benefits - this is called your initial eligibility period. Assurant will allow an additional 15 calendar day grace period to make changes to your enrollment if for some reason you are unable to enroll with in the designated 15 calendar day enrollment period. If you do not enroll by the end of the grace period, you’ll be defaulted into the benefits outlined above.

***You’ll not be able to change your health, dental, vision and Flexible Spending Account elections during the calendar year unless you experience a qualified life event and report it through MyHR within 30 days. Assurant will allow an additional 30 calendar day grace period to make changes to your elections if for some reason you are unable to make your changes within the designated 30 calendar day qualified life event enrollment period.


Qualified Life Event

If you’ve recently experienced a change in your personal, family, or coverage eligibility status, you may be eligible to make certain mid-year changes to your benefits. Be sure to review the information about Qualified Life Events and HIPAA Special Enrollment Rights in the Assurant Health and Welfare Plan Summary Plan Description prior to submitting your change request on MyHR.

The following are examples of qualified life events:

  • Birth, adoption or placement for adoption, or birth of a child from a surrogacy arrangement.
  • Becoming a foster parent/legal guardian.
  • Marriage or divorce.
  • Loss of dependent eligibility.
  • Significant change in your or your spouse’s/domestic partner’s/dependent’s employment status that affects your or his or her eligibility for coverage.
  • Significant changes in coverage under another plan. 

Note: Health Savings Account contributions can be changed at any time on MyHR.

ID Cards

If you enroll in the health plan, you and your covered family members will each receive one combined Anthem Blue Cross and Blue Shield (medical/vision) and CVS Caremark (prescription) card in the mail within 30 days of your enrollment. You also should receive a Health Savings Account (HSA) debit card from Anthem if you enroll in either the Green or Orange plan.

If you need to visit your doctor or use your pharmacy benefits before you receive your new ID cards, you can print temporary ID cards.

To print your Anthem Blue Cross and Blue Shield temporary ID card, first call 1-855-285-4212 to request your ID number. Then, using your ID number, visit Anthem.com to sign up and follow the steps to print off a temporary ID card.

If you enroll in the dental plan, you will not receive a card in the mail and don’t need one. You can print one on the MetLife website.
